Provisions of Program:
• One time payment of the annual fee. No monthly installments
• No refunds on this plan. Not transferable to other offices
• Services and fees are guaranteed for one year
• Initial fee is non-refundable
• Our program is for you and is non-assignable to another person
• One year time limit for completion of all services provided by the program
• Work started before the end of the Program year will be covered
• It is responsibility of the patient to make sure they receive and show up for appointments
• Appointments are required, except for emergency visits. There is a fee for a cancelled appointment without giving advance 24 hour notice
• Only dental procedures listed in the Fee schedule are included
• Referred specialty (By Specialist) work is not included in this plan
• Payment is due at the time service is rendered
